Jason’s look was created by Tom Savini and his team. He originated a plaster mold of Ari Lehman’s head and used that to create prosthetics for his face.
Lehman personally placed mud from the bottom of the lake, all over his body to make himself look really slimy. In part 2, Carl Fullerton only designed a new head for him in one day.
Moreover, Fullerton drew a rough sketch of what he imagined Jason should look like and has it approved. In part 3, Jason wore a mask to cover his face and had worn a bag over his head.

Jason Voorhees is supposed to be a fictitious character and does not exist in real life. But there are striking similarities in the film to a series of grisly murders in Finland in the summer of 1960.
In the series, three teens were stabbed to death while camping at Lake Bodom. Jason was initially created by Victor Miller and was a combined effort by Miller, Ron Kurz, and Tom Savini.

In addition, Voorhees was a disabled child who drowned while trying to swim in Crystal Lake to prove that he could do it. But unfortunately, he died as no one paid attention to him.
The twist was that Jason actually did not die and was hiding in the woods. He lived in a shack in the woods and used to kill anyone who used to come there.
